The exact cause of pilomatrixoma is not yet fully understood. It is a benign subcutaneous tumor in which part of the skin becomes abnormally hard, resembling calcium deposits<\/mark> [1,3].<\/p>\n\n\n\n

\ud83d\udcda Its formal name is \u201cpilomatrixoma,\u201d<\/mark> first described by Malherbe in 1880 as a relatively rare skin tumor [3,19].<\/p>\n\n\n\n

As its alternative name suggests, pilomatrixoma has its origin in hair follicle cells<\/mark>, and mutations in the \u03b2-catenin gene\u2014which plays an important role in hair differentiation\u2014have been reported [5,7,8].<\/p>\n\n\n\n

\ud83e\uddec Activating mutations in the CTNNB1 gene (\u03b2-catenin) are identified in approximately 90% of cases<\/mark>, with abnormal activation of the Wnt signaling pathway considered the underlying mechanism [5,7].<\/p>\n\n\n\n

\ud83d\udc65 Pilomatrixoma lumps most commonly develop in individuals under 20 years of age and tend to occur more frequently in females<\/mark> [4,10,11].<\/p>\n\n\n\n

Large-scale epidemiological studies show that approximately 60% of patients are under 20<\/mark>, with a male-to-female ratio of approximately 1:1.5, indicating a slight predominance in females<\/mark> [4,11,18].<\/p>\n\n\n\n

\ud83d\udcca The overall incidence is estimated at approximately 1\u20132 cases per 100,000 people<\/mark> per year [18].<\/p>\n\n\n\n

\ud83d\udccd Common locations for pilomatrixoma include the face, neck, and arms<\/mark>.<\/p>\n\n\n\n

\ud83e\uddec There is generally no familial inheritance (though multiple lesions on the head may be hereditary and can be associated with myotonic dystrophy) [14].<\/p>\n\n\n\n

In patients with type 1 myotonic dystrophy, multiple pilomatrixomas are reported to occur at approximately 10\u201320 times the rate<\/mark> seen in the general population [14].<\/p>\n\n\n\n

Pilomatrixoma tends to occur predominantly in individuals under 20, with a slight female predominance.<\/p>\n\n\n\n

When pilomatrixoma develops, a stone-hard lump can be felt beneath the skin<\/mark>.<\/p>\n\n\n\n

However, if a pilomatrixoma is extremely large or has poor mobility, it may be difficult to distinguish from a malignant tumor<\/mark>.<\/p>\n\n\n\n

Diagnosis of pilomatrixoma does not begin with imaging; the affected area is first assessed through visual inspection and palpation<\/mark>.<\/p>\n\n\n\n

X-ray imaging can identify the location of the tumor and support a diagnosis of pilomatrixoma; however, in early stages before significant calcification has occurred, the findings may not be clearly visible on imaging.<\/mark><\/p>\n\n\n\n

For a more accurate diagnosis, CT (a medical imaging device that uses X-rays to capture cross-sectional images of the body) or MRI (a device that uses strong magnetic fields and radio waves to create images of internal structures)<\/mark> may be used [6,25].<\/p>\n\n\n\n

\ud83d\udcf8 CT imaging typically reveals characteristic eggshell-like calcification<\/mark>, while MRI shows a mixed signal pattern on both T1- and T2-weighted images<\/mark> [6].<\/p>\n\n\n\n

By visualizing the entire area and identifying the position of the tumor, these imaging modalities help improve diagnostic accuracy.<\/p>\n\n\n\n

Histopathologically, pilomatrixoma is characterized by three hallmark findings: basophilic cells resembling hair matrix cells, eosinophilic shadow cells, and calcification<\/mark> [2,3,19].<\/p>\n\n\n\n

In the pathological diagnosis of pilomatrixoma, the triad of basophilic hair matrix\u2013like cells, shadow cells, and calcification is of central importance<\/mark> [2,3,19].<\/p>\n\n\n\n

Immunohistochemically, nuclear accumulation of \u03b2-catenin is essential for definitive diagnosis<\/mark> and is useful in differentiating pilomatrixoma from other skin tumors [5,8].<\/p>\n\n\n\n

Additionally, CD34 positivity and cytokeratin negativity<\/mark> are also recognized as characteristic findings [19].<\/p>\n\n\n\n

\u26a0\ufe0f Quantifying the Risk of Malignant Transformation<\/h2>\n\n\n\n

Malignant transformation of pilomatrixoma (pilomatrix carcinoma) is extremely rare, occurring in approximately 0.5\u20132% of all cases<\/mark> [21,22].<\/p>\n\n\n\n

Malignant transformation is typically seen in multiple lesions or large tumors that have been left untreated for a long period; rapid growth and ulcer formation are warning signs<\/mark> [21,22].<\/p>\n\n\n\n

\ud83d\udcca Epidemiology and Incidence of Pilomatrixoma<\/h2>\n\n\n\n

Pilomatrixoma is a relatively rare skin tumor, accounting for approximately 0.5\u20131% of all skin tumors<\/mark> [18].<\/p>\n\n\n\n

The annual incidence is estimated at approximately 1\u20132 cases per 100,000 people<\/mark>, with the highest occurrence during childhood and adolescence<\/mark> [10,11,18].<\/p>\n\n\n\n

\ud83d\udccd The head and neck region accounts for approximately 70%<\/mark> of cases, followed by the upper limbs and trunk [12,15].<\/p>\n\n\n\n

\ud83e\uddec Molecular Biological Mechanisms of Pilomatrixoma<\/h2>\n\n\n\n

Abnormal activation of the Wnt signaling pathway<\/mark> plays a central role in the pathogenesis of pilomatrixoma [5,7,8].<\/p>\n\n\n\n

Approximately 90% of cases show activating mutations in exon 3 of the CTNNB1 gene<\/mark>, leading to abnormal accumulation of \u03b2-catenin protein and disrupted differentiation of hair matrix cells [5,7].<\/p>\n\n\n\n

These molecular biological findings provide important insights for improving diagnostic and therapeutic strategies<\/mark> [8].<\/p>\n\n\n\n

\u2728 Post-Treatment Course and Long-Term Prognosis<\/h2>\n\n\n\n

With appropriate surgical excision, the prognosis is favorable; the recurrence rate after complete excision is less than 1%<\/mark> [17,18].<\/p>\n\n\n\n

Cosmetic satisfaction following surgery is generally high, with scars becoming less noticeable over time<\/mark> [6,25].<\/p>\n\n\n\n

An epidermoid cyst is a tumor in which a lump beneath the skin gradually enlarges over time.<\/mark><\/p>\n\n\n\n

It begins as a small sac-like structure that slowly grows as keratin and sebum accumulate inside.<\/mark> Because it also presents as a subcutaneous lump, it can be difficult to distinguish from pilomatrixoma.<\/p>\n\n\n\n

\u26a0\ufe0f If bacteria enter through the small opening on the surface of the cyst and cause infection, symptoms such as pain and redness may develop<\/mark>. As the condition progresses, pus may discharge from the affected area, so prompt medical attention is recommended when an epidermoid cyst is suspected.<\/p>\n\n\n\n

A lipoma is a condition in which fat-storing cells proliferate, causing a swelling or lump to form in the skin.<\/mark><\/p>\n\n\n\n

\ud83d\udccf Lipomas can range widely in size, from approximately 1 cm to 10 cm,<\/mark> and are generally not painful. Most lipomas develop in the subcutaneous fat, which forms the deepest layer of the skin<\/mark>. Like pilomatrixoma, lipomas typically cause no visible changes to the skin surface, making self-diagnosis difficult.<\/p>\n\n\n\n

The precise cause of lipomas is not yet fully understood. However, chromosomal abnormalities are observed in nearly 80% of cases<\/mark>, suggesting that genetic factors may be involved in their development.<\/p>\n\n\n\n

\ud83d\udccc Steatocystoma<\/h3>\n\n\n\n

A steatocystoma is a lump that develops around the sebaceous glands, which secrete sebum. It most commonly appears on the chest, underarms, and neck.<\/mark><\/p>\n\n\n\n

Steatocystomas are generally asymptomatic. If a cyst ruptures and becomes inflamed, pain and swelling<\/mark> may occur.<\/p>\n\n\n\n

Pilomatrixoma tends to grow slowly, but it does not disappear on its own, and if a bacterial infection develops, the area may become red and swollen.<\/mark><\/p>\n\n\n\n

\u274c The tumor cannot be eliminated with oral medications, topical treatments, or laser therapy.<\/mark> Surgical removal is therefore necessary to address the condition.<\/p>\n\n\n\n

Pilomatrixoma usually occurs as a single lesion; however, it is known to develop as multiple lesions in patients with type 1 myotonic dystrophy<\/mark> [14].
In such cases, the incidence has been reported to be 10\u201320 times higher than in the general population<\/mark>, and multiple pilomatrixomas are recognized as an important skin manifestation of this hereditary condition [14].
In general, once a lump is surgically removed, recurrence at the same site is unlikely.<\/mark>

\u26a0\ufe0f If you are experiencing multiple pilomatrixomas, there is a possibility that you may also have a condition called myotonic dystrophy.<\/mark>
Myotonic dystrophy is a hereditary condition in which muscle stiffness leads to progressive muscle weakness. It can affect other organs and, in some cases, cause pilomatrixomas to develop on the scalp.<\/mark>
When pilomatrixoma is caused by myotonic dystrophy, multiple tumors tend to develop.<\/mark> Malignant tumors may also be seen in patients with myotonic dystrophy, so if you have any concerning symptoms, we encourage you to consult a clinic promptly.<\/p> <\/div>

Q. Will there be a scar after surgery?<\/strong>

\u2705 A scar with some redness may remain after surgical excision of pilomatrixoma, but it tends to fade over time.<\/mark>
The scar typically appears as a thin line or crease, and its size will vary depending on the location and size of the tumor.<\/mark>
However, patients with a keloid tendency\u2014where scars become raised and resemble a welt\u2014should be aware that scarring may be more noticeable.<\/mark><\/p> <\/div>

Q. Can babies and children undergo surgery?<\/strong>

\u2705 Surgery for pilomatrixoma can be performed on babies and children without issue.<\/mark>
As the condition progresses, a larger incision may be required, so earlier removal tends to result in a better cosmetic outcome.<\/mark>
\ud83d\udc76 Since it can be difficult for babies and young children to remain still during surgery, general anesthesia is typically used for patients up to around 12\u201313 years of age.<\/mark> Because pilomatrixoma is located close to the surface of the skin, deep incisions are not required, and the physical burden on the patient is generally manageable.
Surgical removal of pilomatrixoma under general anesthesia in babies and young children is not an uncommon procedure, so please do not hesitate to consider an early consultation.
